Rsi+avg With Rsi Upper And Lower Range

Can anybody help me in creating new study using Formula wizard or otherwise.

I know how to add two moving average on RSI. However, i do not know how to display 5 bands on rsi i.e.

1. at 85 level
2. at 40 level
3. at 65 level
4. at 25 level
5. at 50 level

I want this study to display the above 5 bands with rsi with two moving average.

Any help is appreciated.
