There is a one time $195 fee and a recurring $20/month to add the Desktop API to your account (as a developer). The only other fee would be if you wanted to release an application you developed to end-users, in which case you would need to have it certified by eSignal. End-users (i.e. non-developers) can add the ability to run eSignal certified applications for free.
